Although Karky you haven't updated your log in a while, I noticed you did a lot of singles/doubles on the bench. Was this cause of your prior mentioned injury or is this how you work out? Mind sharing it with us, the reason why? Don't you use pyramids or what is your rep scheme?
 
I usually go for a weight that I can do some singles/doubles with, then I try to increase the reps to doing triples consistently, maybe even 4 reps, then I upp the weight like 5kgs and start over.

my injury happened while I was squatting. Though it probably built up over time, as a lot of injuries do.
