Hypertension and Irregular Heartbeat

I'm 6'0'' and weigh 350 pounds. I have high blood pressure (it was classified as malignant hypertension in 2011, but a 3 drug combo successfully lowered it into the realm of moderate hypertension). I also have an irregular heartbeat.
I don't have any idea what sort of foods I should be eating and in what amounts. I've asked my physician and she really wasn't much help. She just said more fruits and vegetables. That's way too general.
I enjoy walking and I can walk pretty far without tiring. I just need a gameplan of some sort. I'm great at following a schedule if I have one. I just can't make one on my own and I can't find any qualified dietician to help me. So I'm asking anyone for advice, ideas, anything.
My weight is killing me. That much I know with certainty. I can feel it slowing me down, making my medical conditions worse.
 
Go through your exercise routine with your doctor regularly. This is particularly important if changes have been made in your medications. Ask your doctor about how any medication changes may affect you and your body's response to exercise before continuing a regular exercise program. New medications can greatly affect your response to activity.
