Frequency of changing routines?

How frequently should I change my weight lifting routine? Should I constantly change it up for "muscle confusion" or should I keep at it until I hit a plateau?

Everyone's different...but...a reasonable guideline is every 2 months, change things up. Whether that means set and rep counts (i.e. varying intensity and volume) or simply different exercises or an alteration to the split.

Muscle confusion is just a fancy P90x term for the fact that muscles adapt. Its not so much 'confusion' as simply the effort to keep challenging the body so you don't plateau.
 
If your general goal is the same and you're still progressing without injuries, unless you've lost motivation in the routine there's no real need to change the routine. On the other hand, if your goals or attitude change, you get injured, or the program just isn't achieving anything anymore, then it may be time to change programs.
