15 y/o, need workout routine & diet

I've been trying to get into a routine for a while now, but I just don't know what I'm doing. I need to draw up a workout routine and diet for myself, and it would be great if you guys could help.

I'm 15 years old, 6'5", and I have back pain when doing anything but tame movements. I'm lanky by nature - and I'd like to put on some muscle. I'm not trying to get fit for any sport, but I might consider taking shaolin kungfu back up if this goes well.

I have a horrible diet right now (Although it doesn't seem to affect my weight. I've been around 68kg for a year now) and I know I have to improve on that whilst attempting to get fit.

One last note, I'm a total novice when it comes to anything diet or fitness related, so please pack your responses with detail (e.g. Tell me exactly what foods I need to eat, and what exercises I need to perform).
